By signing this Waiver and Release of Liability (“Waiver”), I acknowledge and agree as follows: 1. Acknowledgment of Activities I am voluntarily participating in social golf events and related activities organized by The Peach Tree at Approach Shots Indoor Golf (the “Facility”), which may include: • Use of indoor golf simulators and equipment • Physical movement associated with golf activities • Social interaction in a bar or lounge environment • Consumption of alcoholic and non-alcoholic beverages served by the Facility 2. Assumption of Risk I knowingly and voluntarily assume all risks, known and unknown, foreseeable and unforeseeable, associated with participation in these activities, including but not limited to: • Slips, trips, or falls • Being struck by golf balls, clubs, or simulator equipment • Equipment malfunction • Injuries caused by my own actions or the actions of others • Risks associated with alcohol consumption These risks may result in personal injury, illness, property damage, or death. 3. Release of Liability (Georgia Law) To the fullest extent permitted by the laws of the State of Georgia, I hereby release, waive, discharge, and covenant not to sue: • The Peach Tree, its owners, organizers, volunteers, and representatives; and • Approach Shots Indoor Golf, its owners, officers, employees, agents, and affiliates from any and all claims or liability arising from my participation, including claims based on ordinary negligence, but excluding gross negligence or willful or wanton misconduct, which cannot be waived under Georgia law. 4. Alcohol & Age Acknowledgments
I confirm that I am twenty-one (21) years of age or older as of the date of this event and understand that valid identification may be required by the Facility.

I acknowledge that alcoholic beverages may be available at this event. I understand that alcohol consumption increases the risk of injury, impaired judgment, and accidents. I voluntarily assume all risks related to alcohol consumption, agree to drink responsibly, and accept full responsibility for my actions and behavior. I understand that I may choose not to consume alcohol and that participating while impaired may increase my risk of injury.

5. Medical Responsibility I certify that I am physically and medically able to participate. I assume full responsibility for any medical expenses resulting from participation and consent to emergency medical treatment if necessary. 6. Indemnification I agree to indemnify and hold harmless The Peach Tree and Approach Shots Indoor Golf from any claims, damages, losses, or expenses (including reasonable attorneys’ fees) arising from my actions, misconduct, or violation of Facility rules. 7. Photo & Media Release I grant permission to The Peach Tree and Approach Shots Indoor Golf to use photographs or video recordings of me taken during events for marketing and promotional purposes, without compensation. 8. Governing Law This Waiver shall be governed by and construed in accordance with the laws of the State of Georgia. 9. Severability If any provision of this Waiver is held unenforceable, the remaining provisions shall remain in full force and effect. 10. Acknowledgment & Signature
I acknowledge that I have read and understand this Waiver, including the alcohol and age acknowledgments above, and that I am signing it voluntarily.
